CME certificate requests cannot be processed without the posttest and evaluation form. Participants must receive a score of 70% or better to receive credit. Materials must be received by July 31, 2008. After this date the activity will no longer be designated for credit. A CME certificate will be mailed within 6-8 weeks. It is recommended that you keep a copy of your completed materials until you receive your certificate. Please note that you may be contacted regarding a post activity survey.
